Assessment and case coordination

Specialist staff provide comprehensive assessment of new clients health needs in order to determine the most appropriate care plans.
Assessment Officer
The Assessment Officer performs formal client assessments for every new client referred to our organisation, providing support and education to clients and carers in relation to care needs. This role also involves providing nursing care to clients and being an advocate for clients, carers and staff.
Nursing Team Co-ordinators

Team Co-ordinators manage all clinical nursing staff, supporting effective communication between nursing staff in the field and the organisation's Management Team and participating in the organisation's management process by providing advice to the Management Team as requested.

Team Coordinators accept referrals from the Assessment Officers and allocate to the appropriate team member's. They organise team meetings to discuss workload management, client allocations and case discussions as necessary. Team co-ordinators also manage the Client Management System, rostering and daily work planning.

Customer Liaison Officer
The Customer Liaison Officer is responsible for accepting all referrals, updating the client database as appropriate, making appointments for nursing assessments and acts as a go between for clients and nursing staff, relaying messages, emails and correspondence and GP/other providers.
